The Lamenter's Mask can be found in Lamenter's Gaol. It is received as a reward from defeating the Lamenter.
This optional boss can be found deep within Lamenter's Gaol after the player acquires the Lower Level key to descend to his room.
Lamenter Location and How to Beat
As it's in-game description suggests, it can only be used while disrobed. This means, you'll have to unequip all four of your armor slots to achieve the transformation.
Talismans, Weapons, and other Consumables can still be part of your equipment, but all clothing needs to be removed so that you can use the Lamenter's Mask.
Note that you cannot wear armor after transforming either. Head, Body, Legs, and Hand armor all need to be unequipped for the duration of the transformation.
Once used, your character will take on the full appearance of a Lamenter Omen, a disfigured stone creature with curved horns and mangled lesions. This transformation lasts until your next death. You can only revert to your Tarnished appearance after respawning.

To make up for the lost stats provided by your Armor, using the Lamenter's Mask will give you a boost to both your Physical and Magical defenses.
This stacks with any defensive boosts you have from equipped Talismans, and the Damage Negation buffs you get from collecting Scadutree Blessings.

While in the Lamenter Omen's transformation, consuming the Lamenter's Mask again lets you execute an ability where your head enlarges and deals damage in the direction you are facing. It does not require any FP to use and can be useful as a way to interrupt low-damage attacks and knocking back nearby low-poise enemies.
Mind that it is not as effective against opponents with high poise as they can shrug off the knock back.
