To get into Siofra River, take the Siofra River Well elevator near the Minor Erdtree in Mistwood, Limgrave. From there, head straight east, ride another elevator up, and keep on heading east till you reach a wide open space. The first map fragment can be collected from a corpse at the bottom of some steps that lead to Hallowhorn Grounds.

One way to access this area would be to progress through White-Faced Varre's questline. Use the Pureblood Knight's Medal you get from Varre to teleport to Mohgwyn Palace. The map fragment for this area will be on a corpse in the pavilion right in front of you.
Another way to get to Mohgwyn Palace would be to head to the western point of the Consecrated Snowfield of the Mountaintops of the Giants. Defeat the invading Sanguine Noble and find the waygate to the northwest of Yelough Anix Ruins. This waygate will teleport you to Mohgwyn Palace. The map fragment will be at the other side of the island from where you teleport to, so make your way to the west through the blood lake.

Teardrop Scarabs are scurrying little creatures that carry rare and useful items and skills. Crimson and Cerulean Scarabs restore either your HP or FP. Make sure to defeat them before they run away and disappear!
