There are two ways to reach the Siofra River and Nokron, the Eternal City. You can access Siofra River early on by taking the Siofra River Well elevator near the Minor Erdtree in Limgrave.
However, the Eternal City of Nokron can only be accessed after defeating Starscourge Radahn and going down the massive crater left by the meteor that hits Limgrave.
Siofra River Map and Region Guide
There are two options to reach Deeproot Depths. The earlier option is to defeat the Valiant Gargoyles in Siofra River and use the coffin to climb up the waterfall.
The second option is later in the game. It is through two hidden walls at the very bottom of the Subterranean Shunning-Grounds below Leyndell, Royal Capital.
Deeproot Depths Map and Region Guide
There are multiple ways to reach Ainsel River. The easiest way to reach this area and get the map is through the Ainsel River Well, located east of Liurnia of the Lakes.
The rest of the area, including Nokstella and the Lake of Rot, requires you to either reach Deeproot Depths or progress Ranni's questline.
A coffin at the western edge of The Nameless Eternal City in Deeproot Depth can be interacted with and will transport you to the northeast section of Ainsel River. The same area can be reached by passing through the waypoint in Renna's Rise after progressing Ranni's questline.

Mohgwyn Palace can be accessed by a waypoint in the Mountaintops of the Giants or through White Mask Varre's questline.
The waypoint that takes you to this region's eastern part is the Mountaintops of the Giants, northwest of Yelough Anix Tunnel.
Progressing through White Mask Varre's questline also unlocks the ability to teleport directly to the palace outskirts with the Pureblood Knight’s Medal.
