Throw a Throwing Knife through the Window

Head to the eastern side of The Great Bimaristan building and climb all the way up to the rooftop.
Find the barred door and head to the opposite side. Climb on the railing and use your throwing knife to destroy the wooden beam on the barred door. Use Eagle Vision if you cannot find the area to throw the knife at.
Head inside and find the bookshelf on the right side and pull it all the way out of the wall to reveal the gear chest.
