Malus Thorm is a boss that you can encounter at the House of Healing in Baldur's Gate 3 (BG3). Read on to learn how to beat Malus Thorm, where to find him, as well as the drops and rewards that you can get from beating him!

You can skip the entire boss fight by passing two Charisma dialogue checks to persuade Malus Thorm to have his nurses ”practice” their surgical operating skills on him—this results in the nurses stabbing Malus Thorm until he dies!
To convince Malus Thorm to volunteer himself in his own surgical experiment, choose the following dialogue choices in this order:
Failing any of these skill checks will result in open combat with him, so be sure to have enough Inspirations to reroll again should you fail to meet the DC.
Another option would be to convince him to let his nurses turn on each other by selecting the ”Why not have them hone their skills on each other?” which has a DC of 21.
After this bloody segment, you'll be given a choice to fight Malus Thorm without the help of his nurses by his side, or persuade him to kill himself instead. The latter would yet require another DC of 21 to pass.
Failing any of these checks will also lock you in battle with Malus.
Malus Thorm is resistant to non-magical Slashing, Piercing, Bludgeoning, and Fire attacks. Therefore you'll have to cast out spells or any magical skills to deal decent damage to him.
Have a party member with the highest HP and the strongest defense go upfront to distract Malus and tank his attacks, while the rest can spam their spells on him from a reasonable range.
You're free to use any spells except those that yield the element of Fire.
If you have Shadowheart with you or a Cleric in your party, use Guiding Bolt or Sacred Flame as it seems to deal a huge amount of damage to them.
Focus on eliminating the nurses as they can hand out instruments (weapons) to Malus Thorm which changes his attack styles that can impose different debuffs on you.
A great tip is to use Shadowheart's or a Cleric's Turn Undead spell on the nurses so they can't use any Actions, meaning they would not be able to hand over instruments to Malus.
Take note that Malus can revive nurses, so eliminate as many as you can as resurrecting nurses takes up one Action turn for Malus.
If you have Drow Poison in your inventory, throw it at Malus Thorm to poison him and make him fall asleep during the first round.
This gives you enough time in taking out the nurses before dealing with Malus himself.
Malus Thorm can be found in the Surgery Room at the House of Healing which is northwest of the Moonrise Towers.
You can find him in the middle of the room along with his nurses performing a surgical procedure on a patient.
Upon defeating Malus Thorm, you can loot a Surgeon's Subjugation Amulet and a few alchemical ingredients from his corpse.
Nurses will also drop the weapons they were using during the fight. Each weapon has a +1 weapon enchantment bonus and a unique action ability.
